Based on the Stephen King novel of the same name, this is a fairly faithful, and very good adaptation.

Kathy Bates plays Annie Wilkes, a mad old spinster utterly obsessed by the writings of novelist Paul Sheldon, and in particular his character Misery Chastain and the series of books she appears in.
The novelist has the misfortune of suffering a car accident, and it is Annie who saves him, taking him back to her isolated home to nurse him back to health. Thing is, she doesn't want to let him leave, and when she finds out he plans to kill off Misery in his next novel, it's fair to say she flips her lid.

Good performances from the two leads, and with some wince inducing moments - the hobbling being the most notorious - this is quality Hollywood horror.
Like this more than I expected.
